Request Credit

Upon receipt of this completed form, the KCUMB CME Committee will review the proposed program for approval of continuing medical education credit. If approved by the KCUMB CME Committee, the KCUMB CME Department will take responsibility for notifying the Program Director of Committee approval and will submit to the appropriate accrediting agencies for CME approval.

A completed “Request for Designation of CME Credit” form in addition to the attached items requested below must be submitted and received by the KCUMB CME Department at least five working days prior to the scheduled monthly KCUMB CME Committee meeting and no less than 30 days prior to the proposed program date.
